Sun River Health is looking to hire an accomplished full-time Physician (IM/FM) to join our Urgent Care team for our Patchogue, NY site. The Physician is responsible for managing a diverse patient panel with multiple co-morbidities in a busy Federally Qualified Health Center. Care is delivered in collaboration with advance practice providers, on-site support staff, with an emphasis on preventative health and quality patient care.
Essential duties and responsibilities for a Physician:

• Participate in in planning and developing health care services, treatment of conditions and illnesses within scope.
• Diagnose and provide treatment plans for patients.
• Collaborate and supervise advanced provider clinicians.
• Complete and participate in chart review process.
• Referral and follow-up of patients when needed.
• Collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.

Minimum Education Requirement:

• Doctorate Degree (MD, DO)

Licensure/Certification/Registration:

• Active/Valid license as a MD or DO in New York State, DEA, Board certification

Specialized Skills & Knowledge:

• Bilingual in English and Spanish (preferred)
• Experience in HIV care and MAT (Medication Assisted Treatment) (preferred)
• Demonstrated ability to work with diverse patient population in community health setting.

Full time salary range: $200,000 - $220,000
